Banner Health in Northern Colorado earns highest recognition as Age-Friendly Health System
GREELEY, Colo. (Aug. 29, 2024) – Banner Health in Northern Colorado was recognized by the Institute for Healthcare Improvement (IHI) after achieving recognition as an Age-Friendly Health System, earning Committed to Care Excellence status.
Over the last three months, Banner Health has collected data about the count of older adults who received a set of evidence-based elements of high-quality care, known as the 4Ms: What Matters, Medication, Mentation, and Mobility.
Age-friendly health systems aim to: follow an essential set of evidence-based practices; cause no harm; align with What Matters to the older adult and their family caregivers.
“Banner Health has always been committed to care excellence and is proud and humbled to have achieved this recognition,” said Victoria Cordova, director of behavioral health services for Banner Health in northern Colorado. “The Age-Friendly Health Systems initiative continues to be an important part of our overarching vision to provide every older adult with the best care possible. Above all, we aim every day to earn recognition of excellence from the older adults and carers whom we serve. This partnership with our community is our guiding star.”
As of June 2024, more than 2,000 hospitals, practices, convenient care clinics, and nursing homes have been recognized as Age-Friendly Health Systems – Committed to Care Excellence.
“I want to express my gratitude to the team at Banner Health for your dedication to age-friendly care,” said Leslie Pelton, MPA, Vice President, Institute for Healthcare Improvement (IHI). “Age-Friendly Health Systems and IHI celebrate your recognition as an Age-Friendly Health System — Committed to Care Excellence. Because of your efforts, more older adults are receiving safe, high-quality care that is based on what matters most to them as individuals — their specific goals and preferences. And, we can learn from the work you are doing to help inform others across the globe. Thank you for making this happen.”
